Title: Partnering to Effect Health System Change

Synopsis:
Since 2006, Access to Care (ATC) has been a recognized provincial leader in transforming Ontario’s healthcare landscape through the design, implementation and management of provincial IM/IT initiatives that support the Ministry of Health and Long-Term Care. 
This session will provide an overview of the ATC implementation framework: key components and its effectiveness in being applied to successful province-wide health system change initiatives.  
The implementation framework is rooted in key change and project management principles but naturally allows for flexibility; it can be applied to large, complex, multi-stakeholder implementation project, yet can be used in its simplest form to lay out a single-hospital implementation.    ATC is also unique in that it offers a tightly integrated service model to all hospitals that provides both guidance and support to meet the objectives of our mutually aligned goals, and then continues to support those hospitals through ongoing operations.

• If you have strong plan – executing it is easy. 
• If you make it easy for people to complete the work – they’re more apt to complete it – even among other work. 
• Through visibility and transparency – key stakeholders are aware and informed - and stronger relationships are formed through openness and establishment of trust. 
• Ensuring clinical focus and relevance will increase buy-in, acceptance and support towards a successful outcome. 
• Supporting change with a continuous connection leads to on-going success

Bio:
Lianne Bach is the Group Manager, Service Delivery and Management at Cancer Care Ontario. 

With more than 18 years of progressive project management and leadership experience, Lianne specializes in public sector service delivery with a successful track record of provincial deployment initiatives under both Ontario’s Wait Time Strategy and ER/ALC Strategy. In her current role as Group Manager of Service Delivery & Management for Access to Care at Cancer Care Ontario, she works with Access to Care and healthcare teams to guide them through the entire deployment process from inception to implementation and on-going operations. Her experience in strategy development, project management, technical integration, change management and support services is extensive. This experience allows her to design and deliver a framework that is repeatable, straight-forward and intuitive, enabling healthcare teams to achieve sustainable health system change.
